Nature of Services

 

Elizabeth L. Stivers, LCSW, provides psychotherapy for individuals, couples, families, and groups. Therapy is a collaborative process that requires active participation. While many clients find therapy beneficial, outcomes vary, and specific results are not guaranteed.

 

Confidentiality & Limits

 

All client information is protected under HIPAA and professional ethical standards. However, confidentiality has legal exceptions, including:

• If you are at risk of harming yourself or someone else.

• If there is suspected abuse or neglect of a child, elder, or vulnerable adult.

• If a court orders the release of records.

• If insurance providers require information for reimbursement.

2. Privacy Policy (HIPAA Compliance)

Your privacy is a priority. Records are stored securely and only shared with your written consent, except when required by law. You have the right to access your records with a formal request, though certain limitations may apply.

3. Fees, Payment Policies, & Insurance

 

Rates & Payment

 

• Standard session fee: $150 per session.

• Payment is due at the time of service.

• Accepted payment methods: credit/debit card, HSA card, cash, check, Venmo, Zelle.

 

Late Cancellations & No-Shows

 

• Appointments must be canceled at least 24hrs in advance to avoid a $50 fee.

• No-shows may be billed at the full session rate 

 

 

4. Communication & Technology Use

 

Email, Text, & Phone Communication

 

• Email and text are not secure for discussing clinical matters.

• Response time for non-urgent messages is up to one business day.

 

Telehealth Services

 

• Provided via VSee for residents of Tennessee and Maine only.

• Clients must ensure a private and secure environment for sessions.

 

 

5. Cancellation & Refund Policy

 

• No refunds are issued for completed services.

 

6. Emergencies & Crisis Situations

 

This practice does not provide crisis services. In an emergency, please:

• Call 911 or 988

• Contact the Tennessee Crisis Line at 855-274-7471.

• Visit the nearest emergency room.

 

 

7. Ethical & Legal Responsibilities

 

• This practice follows the NASW Code of Ethics and Tennessee and Maine social work laws.

• Services do not include legal, medical, or forensic evaluations.
